Root rot can happen very easily in soil. If over watered, not good drainage, ect. Thats where your confusion begins. All three main growing mediums can get root rot.
Coco and soil as for the for the mentioned above.
Hydro if too warm reservoir, no new fresh air , ect..
They can happen in all for different reasons.
If you are in soil I would say overwatering(from too much at once or too often), no perlite for drainage or not enough drainage holes from pot.
As for treating it you have to transplant.
Bigger pots fresh soil and cut the rotten roots off as much as you can withouth killling healthy roots when transplanting. Make sure new container has holes, perlite, drainage, drainage.!

I agree with UrbZ.
I grow in super soil and compost. I use 5 gal fiber pots, but have used 2 gal and graduated up to 3 gal black plastic landscape pots. They are pretty cheap at the big box stores in the garden dept. They have plenty of holes. I use a bottom layer of super soil, (kind soil), and then compost mixed with perlite to make sure drainage is adequate (about 80% soil to 20% perlite). Don't water until you lift the pot and it is light, or if you stick your finger to the first knuckle and feel no moisture. I have short fingers so I don't water until I can feel no moisture when I stick my finger all the way in the dirt. Or I can feel the bottom of the fiber pot and if it's dry, then it needs water. If you water to run off, don't let the pot sit in the run off, let it soak it up or dump the excess water if it sits around for a few hours. I think you are supposed to ph your run off water, but I don't water to run off so I am not used to doing this.
Have your bigger, new pots, soil and everything you need ready when you go to re-pot your plants, do what UrbZ said and cut off the rotten roots without damaging the good ones.
Try to use the best organic compost without any fertilizer in it, just worm casings and mix it with the perlite very well before adding it to the pot. I use a garbage bag to make sure that it's all mixed up evenly, put a layer in the bottom of the pot, put the plant in, and finish filling the pot with soil. Normally I will water slightly and let the soil settle and the next day go back and see if it needs more water. But that's just me. We all have our different ways to get the result we are looking for.
Good luck!
Current grow:
2-300w led lights
1- 60w cfl for germination
2- 5 gal fiber pots
Kind Soil 5lbs per 5 gal pot
Compost (80%)/perlite (20%)
Seeds:
Shaolin Master auto/fem Harvested
Kush auto/fem (planted 5/13/18) Harvested
Sweet Soma auto/fem ready to go into her home soon (kind soil)
Northern Lights auto/fem, put in soil today, waiting for forever home soon.

Thanks urb and blue for your comments I know exactly where I went wrong I have never ever watered from the bottom a buddy of mine said if you water from the bottom your roots will run down to the water you get better roots I've had a couple good grows great grows actually and I've always water from the top till it's light and then I rewater it I was letting it sit in there overnight till it's soaked it up boy was I wrong there now I'm in the flowering stage right now I'm about 2 weeks almost 3 weeks into flowering I've got all brand new soil lava rocks perlite vermiculite worm castings I got everything I need I just didn't think that I could go ahead and start cutting Roots during this time what's your thoughts on that

I'm only on my third grow, and have gardened my whole life.To keep my plants from dying, I think I would carefully transplant to save the farm! You don't have to have the roots exposed for too long in order to clip the rotten ones off. Just work fast and efficiently and you should be okay. I'd go easy on the vermiculite, it might make your problem continue.
Perlite and vermiculite are both good at retaining water, but vermiculite acts more like a sponge, holding much more water than perlite and offering less aeration for the plant roots. Perlite retains water because of its large surface area with nooks and crannies available for water storage. Because it is porous it allows excess water to drain more readily than vermiculite and improves soil aeration. This was taken from http://homeguides.sfgate.com/vermicu...ite-49660.html.
I just always use perlite myself. Even when gardening in my yard and growing flowers in my flower beds. If you're nervous about having the healthy roots exposed for any length of time, you could cover them with a moist towel, cheesecloth, or even a wet paper bag to keep the air off of them until you get finished with the trimming of the rotten roots.
Lots of luck, and in my case I'd eenie, meenie, miney, moe my decision. It's how I make all of my major decisions in life!

Go easy take of a minimum of good roots off as possible. You will be hindered a bit in the flowering process but not bad if you are gentle. Make sure once you transplant you dont water until the whole thing is dry like you originally were. Maybe go to your local grow store and get some goodies for the roots. Benificial bacteria and enzymes and all. Something similar to Tarantula, piranha and voodoo juice by Advanced Nutes. Doest have to be them but something similar just ask.
